This questionnaire is meant to determine if a new client looking for service is a good fit for BrilliantCARE™.

“On-Boarding” is a process Brilliant AV uses to understand your home and technology systems in order to determine how to best serve you. On-Boarding is only required one time if you are seeking service assistance with your existing systems not installed by Brilliant AV. If you are looking for upgrades or new systems, please use our consultation request instead. To get your On-Boarind Quote we just need to gather some basic information about you and your home systems. Your quote will include everything required for a service technician to fully test and onboard you into Brilliant AV’s one-of-a-kind service program, BrilliantCARE™. If you have not already familiarized yourself with the BrilliantCARE™ program, please do so before you fill out this form.
